First Visit?

If this is your visit to The Rontal Clinic please download and print out this Intake form and be sure to bring it to your first visit with you:

1. All test Results
2. All x-rays, CT scans, and MRIs. Please bring the actual scans, either on film or on a CD. This will greatly help our doctor’s ability to evaluate you.
3. Your doctor’s written referral, if you have one.
4. Your insurance referral, if you have one.
5. A list of all of your medication.
6. Your insurance cards and identification.
7. Your doctor’s address and phone number.
8. Your completed intake forms, if you printed it at home.
