first visit with you:
- All test Results
- All x-rays, CT scans, and MRIs. Please bring the actual scans,
either on film or on a CD. This will greatly help our doctor’s
ability to evaluate you.
- Referral paperwork, if you have any.
- Your insurance cards and identification.
- Your doctor’s address and phone number.
